Getting Started

Scheduling
& Paperwork

Please reach out to me to schedule your first appointment. I am routinely available Monday - Friday from 9 AM - 2:30 PM for sessions online. On Fridays I am available in the South Bend Office.

After we have set a time, I will send you a link to my online portal for you to complete paperwork before we meet. This is a required step, so that I can verify your identity and review your documents before your session.

Visit me for in-person appointments at 218 South Frances Street in South Bend. I am a Friday guest of Freedom Counseling and happy to meet with you in-person there!

Fees

Included with my professional service to you, I will bill your insurance company for counseling. I currently accept the following insurance: 

  • Aetna

  • Anthem BCBS

  • Optum Behavioral Health

  • United Healthcare / Optum

  • Maritain Health / CHA

  • UMR

  • New Avenues / MBHN
     

For clients who would rather pay directly for services without insurance, my discounted fee is $110 for hour sessions (aprox. 55 minutes). I accept cash or credit card. Payment is due at the time of service. 

Appointments must be cancelled at least 24 hours in advance or a $90 fee will apply.​ Unfortunately, without advanced notice I cannot fill your appointment so you will be responsible for that time. (Of course, if we both agree that circumstances were unavoidable, I may waive that fee.)

If you are experiencing financial hardship, ask about limited openings for a reduced fee.

FAQs 

Q: Do you take Medicaid or Medicare?

A: Unfortunately, no, I am not in-network with any Medicaid or Medicare plans.

Q: Is your office handicapped accessible?

A: Yes!

Q: How do I know if I can use my insurance?

A: Before we meet, you will need to call the number on the back of your insurance card and verify if I am a provider you can see. Also ask what your co-pay or deductible may be for mental health care and if you need pre-authorization to see me. 

Q: What payment will I owe?

A: If we have already discussed and verified your insurance, you may only owe me your co-pay at each session (payable by cash or credit card). If you do not have your insurance information available, you will owe the full fee for services. Some clients choose not to file with their insurance carrier and I provide them a discounted rate for this. 

Q: What should I do when I arrive?

A: You can park on the street or behind the building at 218 S Frances. My office is accessed through the front door. Upon entering you will be in a waiting room where you can sit and wait a few moments until your appointment time. The office reads "Freedom Counseling" and I am their guest on Fridays. Other days of the week we meet online virtually.
